Jacklynn Nichole Smith, 1 month old, of 405 W. Legion Blvd., Owensboro, passed away on Monday, August 11, 2008 at her home. She was born in Daviess Co., Ky.
Surviving are her parents, Kimberly M. Phelps and Larry D. Smith, grandparents, Tammy Kennedy, Carroll Phelps, Sherry Phelps, and Larry Whitaker, great grandparents, Mary Frank, Betty Ann Hoyt, and Alfred Chambers, and a sister, Sabrina M. Smith.
Services will be 1:00 P.M. Friday, August 15, at the Christ Chapel at Owensboro Memorial Gardens with burial there following the service.
