Blake Banken, infant son of Juanita Daugherty and Tyler Banken, was born and died on Friday, May 15, 2015 at Owensboro Health Regional Hospital.
Also surviving are his grandparents, Tony and Patrice Banken and Melissa KcKinney, and great grandmother, Karen Vonderheide.
There will be no services. Glenn Funeral Home and Crematory are in charge of arrangements.
