Avery Ruth Phelps, infant daughter of Jeremy and Melissa Phelps, was born July 6, 2006 at Owensboro Medical Health System.
Surviving are her parents, a sister, Hadley Nicole Phelps, at home; her maternal grandparents, Gary and Krista Powers of Hawesville, her paternal grandparents, Philip and Brenda Phelps of Owensboro; her maternal great-grandmother, Ruby Mayfield of Hawesville, her paternal great-grandmother, Betty Drury of Owensboro and her paternal great-grandfather, James Phelps of Beaver Dam; several aunts, uncles and cousins.
Graveside services will be 10:00 AM Saturday at Mater Dolorosa Cemetery. Visitation will be after 9:00 AM Saturday at Glenn Funeral Home.
